I don't claim this will work for everyone, but it appears to have worked for me. So here are my findings.
My setup
Windows 7 (64bit) / 1333Mhz 8GB RAM / 3.5GHz Intel i7-2700K CPU
PeridexisErrant's LNP
TWBT, Liquid Depth, Varied Ground, all ON
Popcap (150/150) Strict Pop Cap (300)
Temperature, Weather, Cave-ins, all ON
Default phoebus (16px),
DFhack's plugins like 'Other Automation Plugins' and 'Performance Tweaks', Dwarf Therapist, and Soundsense.
Embark size 4x4, visible brook with flowing water, trees, medium world, long history (1050 years).
Average CPU FPS (30/50), Average FPS (25/25)
Causes of my crashes
Why and how to download and apply the 4GB patch to any executable file Follow Sim Racing Tech on Facebook: To contact me e-mail. Find NTCore software downloads at CNET Download.com, the most comprehensive source for safe, trusted, and spyware-free downloads on the Web.
The game frequently crashes when it autosaves (seasonally). When I browse the [d]etails for a statue being made to find historical figures, and when I check the [z] stock screen for [e]hanced view. Frequent as in, maybe once every 10 times. And infrequently crashes when I navigate visible ground levels via arrow keys and [<] and [>] for going through levels.
The game freezes for a moment, where nothing moves, then a second later 'dwarf fortress has stopped responding'.
The game also crashes when trying to quit in the main menu after a save had been successful.

Application of NTCore patch
I don't know how the NTCore patch works nor LNP DF and its features, so I applied it to all .exe that I found relevant within the LNP folder.
Dwarf Fortress.exe
dfhack-run.exe
Starter Pack Launcher (PyLNP).exe
Results
It hasn't crashed mid-game yet. After 3 in-game years, playing an in-game year per day, with the same exact setup but with NTCore applied. When it wants to crash (i.e. it hangs a bit after I press [z], browse stocks and press [e], browsing multiple layers and a hiccup that usually contributes to the prior behaviours of a crash), the game would resume after that hiccup. But there was one instance after I saved successfully by in-game menu, and after pressing enter on [quit], when it should exit the game, it crashed instead. So I don't think it'll be perfect.
I found NTCore while looking up the Known Bugs section of the DF Wiki, under bug 136.
How I played previously was to use Ctrl + Alt + S for the quicksave feature of DFHack, that seems to crash the game much less often than using the autosave for me.

TLDR: I applied a patch I found on the bug section of the wiki to all the exe's in LNP and the game stopped crashing as much. Hopefully it'll help your session too.
